Small Cell Lung Cancer
A very fast-growing, aggressive lung cancer strongly linked to heavy smoking, presenting centrally in the chest and often spreading early to the brain and other organs.
Emergency Management: Symptomatic hyponatremia causing seizures requires hypertonic (3%) saline. SVC syndrome with airway compromise requires emergency stenting or radiation.
Small Cell Lung Cancer (SCLC) is a highly aggressive, poorly differentiated neuroendocrine carcinoma of the lung. It is characterized by a rapid doubling time, high growth fraction, and early, widespread metastasis. SCLC is almost exclusively found in individuals with a heavy smoking history.
Detailed Overview
Accounting for about 15% of all lung cancers, SCLC typically originates centrally in the large airways and rapidly invades mediastinal lymph nodes. Because of its neuroendocrine origin, it is notorious for causing paraneoplastic syndromes, such as SIADH and Cushing syndrome. The disease is clinically divided into Limited Stage (confined to one hemithorax and safely treatable within a single radiation port) and Extensive Stage (spread beyond the hemithorax). While extremely sensitive to initial chemotherapy and radiation, SCLC almost inevitably relapses, making long-term survival rare.
Epidemiology & Demographics
Represents ~15% of lung cancers. Declining incidence parallel to decreasing smoking rates. Median age at diagnosis is 65. Almost 100% of cases are current or former smokers.
Etiological Mechanism
Massive genomic instability driven by tobacco carcinogens. Universal loss of function in tumor suppressor genes TP53 and RB1 is the molecular hallmark of SCLC.
Primary Causes
Heavy, prolonged tobacco smoking.
SCLC arises from pulmonary neuroendocrine cells (Kulchitsky cells) in the bronchial mucosa. Carcinogens cause ubiquitous bi-allelic inactivation of TP53 and RB1, removing cell cycle checkpoints and allowing unchecked proliferation. The cells appear small, with scant cytoplasm, ill-defined borders, finely granular nuclear chromatin (salt and pepper), and absent nucleoli. High mitotic rates and extensive necrosis are visible. Because these cells secrete ectopic hormones, paraneoplastic syndromes occur rapidly. Tumor cells rapidly access local lymphatics and the bloodstream, seeding the liver, bone marrow, and brain very early in the disease course.
Diagnostic Criteria & Guidelines
Histological confirmation via bronchoscopic biopsy or FNA showing characteristic small blue round cells with salt and pepper chromatin, positive for neuroendocrine markers (Synaptophysin, Chromogranin, CD56).
For Limited Stage (LS-SCLC): Concurrent chemoradiation with Cisplatin (75 mg/m2 IV Day 1) and Etoposide (100 mg/m2 IV Days 1-3) given every 3 weeks for 4 cycles, plus thoracic RT. Followed by Prophylactic Cranial Irradiation (PCI) if good response. For Extensive Stage (ES-SCLC): Chemoimmunotherapy consisting of Carboplatin + Etoposide + Atezolizumab (1200 mg IV) for 4 cycles, followed by maintenance Atezolizumab.
Second-Line & Adjunctive Therapy
Lurbinectedin 3.2 mg/m2 IV every 21 days OR Topotecan 1.5 mg/m2 IV days 1-5 every 21 days for relapsed disease.
Surgical & Procedural Management
Surgery is almost NEVER indicated for SCLC unless it presents as an isolated solitary pulmonary nodule (T1-2, N0) diagnosed post-resection.
Patient Counseling & Advice
Provide highly realistic counseling regarding the aggressive nature of the disease. Explain that while the tumor initially shrinks rapidly and dramatically with chemo, it almost always comes back. Discuss advanced directives early.
Follow-Up & Monitoring Schedule
CT Chest/Abdomen/Pelvis every 2-3 months for the first 2 years, then every 6 months. High suspicion for brain mets requires repeat MRI Brain on symptom onset.
Preventive Strategies
Do not start smoking. Smoking cessation reduces risk over time but does not eliminate it.
LS-SCLC 5-year survival is 20-25%. ES-SCLC median survival is 10-13 months, with a 5-year survival of <2%.
