Raynaud Phenomenon
Episodic vasospasm of the digits causing color changes (white, blue, red) in response to cold or stress, either benign (primary) or secondary to autoimmune diseases.
Emergency Management: Acute Digital Ischemia: Persistent cyanosis/pallor with severe rest pain for hours. Requires urgent hospitalization, IV Iloprost, and possible anticoagulation or surgical sympathectomy.
Raynaud Phenomenon (RP) is an episodic, reversible vasospastic disorder of the small cutaneous arteries and arterioles, most commonly affecting the fingers and toes. It is triggered by cold exposure or emotional stress, resulting in a characteristic triphasic color change: ischemic pallor (white), followed by cyanosis (blue), and finally reactive hyperemia (red).
Detailed Overview
RP is classified into primary (Raynaud disease) and secondary (Raynaud syndrome) forms. Primary RP is a benign, idiopathic exaggeration of normal vascular responses to cold, predominantly seen in young females. Secondary RP is associated with underlying systemic diseases, most notably connective tissue disorders like Systemic Sclerosis (Scleroderma) and Systemic Lupus Erythematosus (SLE). Secondary RP is often more severe, potentially leading to irreversible tissue damage, digital ulceration, and gangrene, requiring aggressive pharmacological intervention.
Epidemiology & Demographics
Prevalence is 3-5% in the general population. It is much more common in women (female-to-male ratio of 4:1) and typically presents between ages 15-30 for primary RP. Secondary RP presents later in life.
Etiological Mechanism
The etiology is a hyperreactivity of the digital vasculature to sympathetic nervous system stimulation. Primary RP has no underlying cause. Secondary RP is driven by endothelial dysfunction, vascular structural abnormalities, and inflammatory mediators associated with autoimmune diseases.

Normal thermoregulation involves arteriovenous anastomoses (AVAs) in the digits controlled by sympathetic adrenergic nerves. In RP, there is an exaggerated alpha-2 adrenergic receptor response to cold, causing intense vasoconstriction of AVAs and digital arteries. This leads to the pallor phase (ischemia). Depletion of oxygen causes capillary dilation and deoxygenated blood pooling, resulting in the cyanotic phase. Upon rewarming, sudden vasodilation occurs, causing reactive hyperemia (rubor phase). In secondary RP, intimal proliferation, microvascular endothelial injury, and impaired release of vasodilators (nitric oxide, prostacyclin) exacerbate the vasospasm and lead to fixed vascular obstruction.
Diagnostic Criteria & Guidelines
Diagnosis is clinical based on history of cold sensitivity and classical color changes. Differentiation between primary and secondary is via Nailfold Capillaroscopy and autoantibody testing (ANA).
Non-pharmacological measures first. If meds are needed: Dihydropyridine Calcium Channel Blockers. Amlodipine 5-10 mg PO daily or Nifedipine extended-release 30-90 mg PO daily. These act as direct vasodilators.
Second-Line & Adjunctive Therapy
Phosphodiesterase-5 (PDE-5) inhibitors: Sildenafil 20 mg PO TID. Topical nitrates (Nitroglycerin 2% ointment) applied to the base of the digits. For severe secondary cases with ulcers: IV Iloprost (prostacyclin analog) infusion or Bosentan (endothelin receptor antagonist).
Surgical & Procedural Management
Digital Sympathectomy (stripping the adventitia of the digital arteries to sever sympathetic nerve fibers) for medically refractory cases with impending gangrene.
Patient Counseling & Advice
Reassure primary RP patients that the condition is benign and will not cause limb loss. Counsel secondary RP patients to seek immediate care if a finger remains persistently blue or painful for > 30 minutes, or if sores develop.
Follow-Up & Monitoring Schedule
Annual clinical review for primary RP to ensure no development of systemic signs. Rheumatology follow-up every 3-6 months for secondary RP.
Preventive Strategies
Behavioral adaptation is the only true prevention. Avoiding rapid temperature drops and managing emotional stress through biofeedback.
Primary RP has an excellent prognosis, often improving with age. Secondary RP prognosis depends entirely on the underlying connective tissue disease and degree of vascular damage.
