Peripheral Artery Disease (PAD) is an atherosclerotic occlusive disease of the lower extremities characterized by the stenosis or occlusion of the arteries, most commonly the superficial femoral and popliteal arteries. It results in an imbalance between blood supply and tissue demand.

Detailed Overview

PAD is a powerful clinical marker of systemic atherosclerotic burden, carrying a high risk of concomitant coronary artery and cerebrovascular disease. As the arterial lumen narrows due to atheroma formation, downstream tissues experience ischemia, initially during exertion (claudication) and eventually at rest. The progression to Critical Limb Ischemia (CLI) poses a significant risk for tissue loss and amputation. Management focuses on cardiovascular risk reduction, symptom improvement, and limb salvage.

Epidemiology & Demographics

Affects approximately 8-12 million people in the United States. Prevalence strongly increases with age, reaching >20% in individuals over 80. More common in non-Hispanic Black populations.

Etiological Mechanism

Atherosclerosis is the primary cause, driven by endothelial dysfunction, lipid accumulation, and chronic inflammation. Less common causes include fibromuscular dysplasia, vasculitis, and thromboembolism.

Primary Causes

Atherosclerotic plaque accumulation in the abdominal aorta, iliac, femoral, popliteal, and tibial arteries.

The pathophysiologic hallmark is atherosclerosis. Endothelial injury allows LDL to enter the tunica intima, where macrophages engulf oxidized LDL to form foam cells, creating a fatty streak. Over time, smooth muscle proliferation and extracellular matrix deposition form a fibrous plaque that progressively narrows the arterial lumen. When stenosis exceeds 50-75%, distal perfusion pressures drop significantly during exercise, causing a mismatch in oxygen delivery. In advanced disease, resting blood flow becomes inadequate, leading to ischemic rest pain and tissue necrosis.

Diagnostic Criteria & Guidelines

Diagnosis is confirmed by an Ankle-Brachial Index (ABI) ≤ 0.90 at rest. An ABI of 0.91-0.99 is borderline. ABI >1.40 indicates noncompressible calcified vessels (common in diabetes) requiring Toe-Brachial Index (TBI).

First-Line Treatment:

Cardiovascular risk reduction: High-intensity statin (Atorvastatin 80 mg or Rosuvastatin 40 mg daily) and Antiplatelet therapy (Aspirin 81 mg daily OR Clopidogrel 75 mg daily). For symptom relief: Supervised exercise program (minimum 30-45 mins, 3 times/week for 12 weeks) and Cilostazol (100 mg BID) for claudication.

Second-Line & Adjunctive Therapy

Rivaroxaban 2.5 mg BID plus Aspirin 81 mg daily (COMPASS trial regimen) for high-risk patients to reduce cardiovascular events and major adverse limb events.

Surgical & Procedural Management

Endovascular revascularization (percutaneous transluminal angioplasty with or without stenting) for short segment lesions. Surgical bypass (e.g., femoro-popliteal bypass using autologous saphenous vein) for long, complex lesions or CLI.

Patient Counseling & Advice

Emphasize that while leg pain is the primary symptom, the highest risk to their life is actually heart attack or stroke. Supervised exercise initially causes pain but is proven to significantly increase maximum walking distance over months.

Follow-Up & Monitoring Schedule

Clinical evaluation and resting ABI every 6-12 months. Monitor lipid profile and HbA1c to ensure targets are met (LDL < 70 mg/dL, HbA1c < 7%).

Preventive Strategies

Aggressive management of hypertension, diabetes, and hyperlipidemia in primary care. Routine ABI screening is recommended in patients >65 years, or >50 years with a history of smoking or diabetes.

Patients with claudication have a 5-year limb loss rate of only 1-2%, but a 5-year cardiovascular mortality rate of 15-30%. If disease progresses to CLI, 1-year amputation or mortality rate approaches 25-30% without intervention.
