Korsakoff Syndrome
A chronic memory disorder caused by severe vitamin B1 deficiency, usually from chronic alcoholism, causing inability to form new memories and making up stories.
Emergency Management: Wernicke encephalopathy is the preceding medical emergency. Failure to treat it promptly with high-dose IV thiamine guarantees irreversible progression to Korsakoff syndrome.
Korsakoff Syndrome is a late, chronic neuropsychiatric manifestation of severe thiamine (Vitamin B1) deficiency, predominantly seen in individuals with severe alcohol use disorder. It is characterized by severe anterograde and retrograde amnesia with relatively preserved sensorium and implicit memory.
Detailed Overview
Korsakoff syndrome almost always follows an acute, often unrecognized, episode of Wernicke encephalopathy (WE). While WE (confusion, ataxia, ophthalmoplegia) is acute and reversible with thiamine, KS represents irreversible structural damage to the diencephalon (mammillary bodies and thalamus). Patients with KS cannot form new memories and often 'confabulate'—unconsciously inventing stories to fill memory gaps. Because insight is severely impaired, patients typically do not realize they have a memory deficit. Institutional care is often required due to profound functional impairment.
Epidemiology & Demographics
Prevalence is estimated at 1-2% of the general population, but much higher (up to 12%) in alcohol-dependent populations. It is often underdiagnosed post-mortem.

Thiamine pyrophosphate is a crucial coenzyme for glucose metabolism (transketolase, pyruvate dehydrogenase, alpha-ketoglutarate dehydrogenase). Severe deficiency leads to cerebral energy failure, lactic acidosis, and glutamate excitotoxicity. This cascade causes microhemorrhages, gliosis, and neuronal necrosis, specifically targeting the mammillary bodies, the anterior and mediodorsal nuclei of the thalamus, and the periaqueductal gray. The destruction of these limbic structures disrupts the Papez circuit, permanently impairing memory consolidation.
Diagnostic Criteria & Guidelines
Clinical diagnosis based on a history of thiamine deficiency/alcoholism, presence of anterograde amnesia, and exclusion of other causes of dementia. DSM-5 categorizes it under 'Substance-Induced Major Neurocognitive Disorder, Amnestic Type'.
High-dose intravenous Thiamine (Vitamin B1) to prevent further damage. Typically 500 mg IV TID for 3-5 days, followed by 250 mg IV/IM daily for another 3-5 days. Always administer Thiamine BEFORE or ALONGSIDE glucose infusions. Subsequent oral thiamine 100 mg daily indefinitely.
Second-Line & Adjunctive Therapy
Supportive psychiatric care and cognitive rehabilitation. No medications are FDA-approved to restore memory in Korsakoff syndrome (cholinesterase inhibitors like Donepezil have minimal efficacy).

Patient Counseling & Advice
Counseling is primarily directed at the family/caregivers, as the patient lacks insight. Caregivers must understand that the patient is not 'lying' when they confabulate, and that the memory loss is likely permanent, requiring long-term structured care.
Follow-Up & Monitoring Schedule
Placement in a memory care facility or structured home environment. Routine monitoring of nutritional status and abstinence from alcohol.
Preventive Strategies
Empiric administration of IM/IV Thiamine to all malnourished, alcoholic, or at-risk patients presenting to the emergency department, especially before giving IV dextrose.
Poor for cognitive recovery. Only about 20% of patients achieve significant, meaningful recovery of memory function with prolonged abstinence and thiamine. The rest require permanent institutional care or close supervision.
