Gastroesophageal Reflux Disease (GERD) is a chronic digestive condition where stomach acid or, occasionally, stomach content, flows back into the esophagus. This retrograde flow irritates the esophageal lining, leading to troublesome symptoms or mucosal damage. It represents a functional failure of the lower esophageal sphincter (LES) barrier.

GERD is one of the most common GI disorders globally. While typical symptoms include heartburn and regurgitation, atypical presentations like chronic cough or laryngitis are frequent. The clinical significance of GERD extends beyond symptomatic distress; persistent acid exposure can induce structural changes such as erosive esophagitis, esophageal strictures, and Barrett's esophagus—a metaplastic change that is a direct precursor to esophageal adenocarcinoma.

Epidemiology & Demographics

Prevalence is 18-28% in North America. Incidence increases with age. Equal gender distribution, though men are at higher risk for complications like Barrett's esophagus (3:1 male to female). More prevalent in Western countries linked to obesity rates.

Etiological Mechanism

The primary etiology is mechanical failure of the anti-reflux barrier. This involves transient lower esophageal sphincter relaxations (TLESRs), a hypotensive LES (< 10 mmHg resting pressure), or anatomic disruption such as a hiatal hernia.

Primary Causes

Obesity (increased intra-abdominal pressure), hiatal hernia, pregnancy, delayed gastric emptying (gastroparesis), and medications that decrease LES pressure (e.g., calcium channel blockers, nitrates).

Normally, the LES maintains a high-pressure zone to prevent reflux. In GERD, frequent TLESRs (not triggered by swallowing) allow gastric acid (pH < 4), pepsin, and bile acids to ascend into the esophagus. The squamous epithelium of the esophagus is vulnerable to this low-pH fluid. This leads to disruption of intercellular tight junctions, triggering an inflammatory cascade (release of IL-8, recruitment of neutrophils) which manifests as erosive esophagitis. Chronic healing and injury cycles promote columnar metaplasia (Barrett's esophagus).

Diagnostic Criteria & Guidelines

Classic symptoms of heartburn and regurgitation responding to an 8-week trial of empiric PPI therapy confirm the diagnosis. If alarm symptoms exist, endoscopy is required. Gold standard for atypical or PPI-refractory cases: 24-hour ambulatory esophageal pH monitoring showing DeMeester score > 14.72 or acid exposure time > 6%.

First-Line Treatment:

Proton Pump Inhibitors (PPI): Omeprazole 20 mg daily or Pantoprazole 40 mg daily, taken 30-60 minutes before the first meal of the day, for an 8-week trial. Titrate to lowest effective dose.

Second-Line & Adjunctive Therapy

For refractory symptoms: Increase PPI to BID dosing (e.g., Omeprazole 20 mg BID). Add H2-receptor antagonist at bedtime (e.g., Famotidine 20 mg PO QHS) for nocturnal acid breakthrough.

Surgical & Procedural Management

Laparoscopic Nissen Fundoplication: Indicated for patients who fail medical management, opt out of lifelong medications, or have large hiatal hernias with volume regurgitation.

Patient Counseling & Advice

Advise on trigger avoidance (fatty foods, chocolate, alcohol, caffeine). Educate about potential long-term PPI risks (e.g., B12 deficiency, bone fractures, enteric infections) but reassure that absolute risks are small.

Follow-Up & Monitoring Schedule

Symptom reassessment after 8 weeks of PPI therapy. Upper endoscopy screening for Barrett's esophagus in men >50 years with chronic GERD symptoms (>5 years) and other risk factors.

Preventive Strategies

Maintaining a normal BMI and avoiding tobacco and excessive alcohol consumption.

Excellent with medication; >80% achieve symptom relief on PPIs. However, symptoms often relapse upon discontinuation, and many require long-term maintenance therapy.
