Dyshidrotic eczema is an intensely pruritic, chronic, and recurrent vesicular dermatitis of unknown etiology that uniquely affects the palms, soles, and lateral aspects of the fingers and toes. It is characterized by the sudden eruption of deep-seated, 'tapioca-like' clear vesicles.
Detailed Overview
Despite its historical name (dyshidrosis), the condition is not a disorder of the eccrine sweat glands. It is a distinctive morphological pattern of hand and foot eczema. It can severely impact quality of life due to intense itching and subsequent painful fissuring and desquamation. Exacerbating factors often include hyperhidrosis, emotional stress, and contact allergens (like nickel).
Epidemiology & Demographics
Accounts for about 5-20% of all hand dermatitis. Peak incidence is in young adults aged 20 to 40. Females are affected twice as often as males.
Etiological Mechanism
Idiopathic, but considered a multifactorial delayed-type hypersensitivity reaction. Often associated with atopy (personal or family history of asthma, allergic rhinitis, atopic dermatitis).
Primary Causes
Triggers include exposure to contact allergens (metals like nickel, cobalt, chromate), irritants (soaps, detergents), dermatophyte infections elsewhere on the body (id reaction), emotional stress, and warm weather.
The precise mechanism is unresolved. It involves a spongiotic inflammatory process in the thick epidermis of the palms and soles. Inflammatory infiltrates (T-lymphocytes) and cytokines cause intercellular edema in the epidermis (spongiosis). Because the stratum corneum in these areas is exceptionally thick, the edema forms deep-seated vesicles rather than rupturing easily on the surface. Later, as vesicles resolve, the stratum corneum dries, cracks, and desquamates.
Diagnostic Criteria & Guidelines
Clinical diagnosis based on history and typical morphology. KOH preparation should be performed to rule out dermatophyte infection (tinea). Patch testing is recommended in chronic cases to identify occult contact allergens.
High-potency topical corticosteroids are the mainstay. Ointments are preferred over creams. Clobetasol propionate 0.05% ointment or Betamethasone dipropionate 0.05% applied BID for 2-4 weeks. Wet compresses (e.g., Burow's solution/aluminum acetate) for 15 mins twice daily help dry out acute weeping blisters.
Second-Line & Adjunctive Therapy
Topical calcineurin inhibitors (Tacrolimus 0.1% ointment) to spare steroids. For severe, acute flares: Systemic corticosteroids (Prednisone 40 mg/day tapered over 10-14 days). For chronic refractory cases: Oral Alitretinoin (30 mg/day), Methotrexate (10-15 mg/week), or Phototherapy (PUVA or NBUVB).
Surgical & Procedural Management
Not applicable, though large tense bullae (pompholyx) may be aseptically drained (punctured) while leaving the roof intact.
Patient Counseling & Advice
Advise the patient not to pop the blisters themselves, as this significantly increases the risk of bacterial infection. Warn that the condition is chronic and relapsing, and strict adherence to hand hygiene modification is as important as medication.
Follow-Up & Monitoring Schedule
Re-evaluate at 4 weeks to assess response to topical steroids and taper down to avoid skin atrophy. Check for signs of secondary infection.
Preventive Strategies
Identify and eliminate specific triggers (e.g., dietary nickel avoidance if proven allergic, stress management). Use barrier creams.
Typically a chronic, relapsing-remitting course over years. Frequency and severity of attacks tend to decrease spontaneously with age.
