Dermatomyositis
An autoimmune disease causing progressive muscle weakness and distinct skin rashes, often linked to an underlying cancer in adults.
Emergency Management: Severe pharyngeal muscle weakness causing inability to swallow requires urgent hospitalization, NG tube feeding for airway protection/nutrition, and high-dose IV pulse steroids + IVIG.
Dermatomyositis (DM) is a rare, idiopathic inflammatory myopathy characterized by immune-mediated symmetric proximal muscle weakness and characteristic cutaneous manifestations. It is frequently associated with systemic manifestations including interstitial lung disease and, notably in adults, a high risk of underlying occult malignancy.
Detailed Overview
Unlike polymyositis, which is primarily driven by cytotoxic T-cells attacking muscle fibers directly, dermatomyositis is a microangiopathy. It is driven by complement-mediated damage to the endothelial cells of the capillaries within the muscle fascicles and skin. This ischemia leads to muscle fiber necrosis (perifascicular atrophy). The disease can profoundly impair daily activities, rendering patients unable to rise from a chair or comb their hair. In 15-25% of adult cases, DM represents a paraneoplastic syndrome, necessitating rigorous cancer screening.
Epidemiology & Demographics
Incidence is roughly 1-2 per 100,000 person-years. Bimodal age distribution: peaks in childhood (juvenile DM, ages 5-15) and adulthood (ages 40-60). Females are affected twice as often as males.
Etiological Mechanism
Idiopathic autoimmune disorder, likely triggered by environmental factors (e.g., viral infections, UV radiation, medications) in genetically susceptible individuals. Paraneoplastic phenomenon in many adult cases.
Primary Causes
Autoimmune complement-mediated microangiopathy.
The primary target in DM is the endothelium of the endomysial capillaries. The classical complement pathway is activated, leading to the deposition of the C5b-9 Membrane Attack Complex (MAC) on capillary walls. This causes endothelial swelling, capillary destruction, and localized ischemia. The muscle fibers furthest from the central blood supply (at the periphery of the fascicle) suffer the most ischemia, resulting in the pathognomonic biopsy finding of 'perifascicular atrophy'. Concurrently, CD4+ T cells, plasmacytoid dendritic cells, and B cells infiltrate the perimysium. Type 1 interferons are heavily upregulated, contributing significantly to the cutaneous inflammation and systemic features.
Diagnostic Criteria & Guidelines
Bohan and Peter Criteria: 1) Symmetric proximal muscle weakness, 2) Elevated skeletal muscle enzymes, 3) Myopathic EMG changes, 4) Characteristic muscle biopsy findings, 5) Typical skin rash (Gottron's or heliotrope). Definite DM = Rash + 3 other criteria.
Systemic Glucocorticoids: Prednisone 1 mg/kg/day PO (or IV Methylprednisolone pulse for severe dysphagia/ILD). To limit steroid toxicity, a steroid-sparing immunosuppressant is started concurrently, most commonly Methotrexate 15-25 mg/week OR Azathioprine 2-3 mg/kg/day. For the skin rash: Strict photoprotection, high-potency topical corticosteroids, and Hydroxychloroquine 200-400 mg/day.
Second-Line & Adjunctive Therapy
For refractory myositis: Intravenous Immunoglobulin (IVIG) 2 g/kg monthly (FDA approved). Rituximab 1000 mg IV x 2 doses (highly effective for Antisynthetase syndrome/ILD). Mycophenolate Mofetil is often preferred for ILD. Cyclophosphamide for severe, rapidly progressive ILD.
Surgical & Procedural Management
None specific for DM. Excision of calcinosis cutis (calcium deposits in skin/muscle, more common in juvenile DM) if severe and limiting function.
Patient Counseling & Advice
Inform the patient that recovery of muscle strength is slow and can take 2-3 months after starting steroids. Strongly emphasize that they must undergo age-appropriate and extensive cancer screening (colonoscopy, mammogram, pap smear, CT chest/abdomen/pelvis) because treating an underlying hidden cancer is sometimes the only way to cure the dermatomyositis.
Follow-Up & Monitoring Schedule
Serial measurements of muscle strength using standardized scales (e.g., MMT-8) and monitoring of CK/aldolase levels. Regular PFTs (DLCO) to monitor for lung involvement. Repeat age-appropriate malignancy screening annually for the first 3 years.
Preventive Strategies
No known primary prevention. Strict sun avoidance prevents disease flares.
The 5-year survival rate is 70-80%. Mortality is primarily driven by underlying malignancy, rapidly progressive interstitial lung disease, or severe aspiration pneumonia.
