Dementia with Lewy Bodies (DLB) is a progressive, neurodegenerative disorder characterized by early, prominent cognitive decline (specifically in executive function and visuospatial abilities), fluctuating cognition, recurrent visual hallucinations, and spontaneous parkinsonian motor features.
Detailed Overview
DLB is the second most common neurodegenerative dementia after Alzheimer's disease. Pathologically, it is defined by the widespread aggregation of alpha-synuclein protein into intraneuronal inclusions called Lewy bodies throughout the cerebral cortex, limbic system, and brainstem. Clinically, it uniquely blends the cognitive phenotype of Alzheimer's with the motor phenotype of Parkinson's disease. A hallmark characteristic is extreme sensitivity to neuroleptic (antipsychotic) medications, which can cause severe, irreversible, and potentially fatal motor worsening.
Epidemiology & Demographics
Accounts for 10-15% of all dementia cases. Affects approximately 1.4 million individuals in the US. More common in men than women. Typical onset is between 70 and 80 years of age.
Etiological Mechanism
The exact cause is unknown, but it is classified as an alpha-synucleinopathy. Most cases are sporadic, though rare familial forms exist linked to mutations in the SNCA or GBA genes.
Primary Causes
Abnormal aggregation of alpha-synuclein protein forming Lewy bodies and Lewy neurites.
Misfolded alpha-synuclein proteins aggregate into insoluble fibrils, forming Lewy bodies within the cytoplasm of neurons. Unlike Parkinson's disease, where Lewy bodies are initially confined to the substantia nigra, in DLB they rapidly disseminate throughout the neocortex (causing dementia) and limbic system (causing hallucinations). This widespread pathology disrupts multiple neurotransmitter systems. There is a profound deficit in cortical acetylcholine (greater than in Alzheimer's), driving the cognitive fluctuations and visual hallucinations. Concurrently, there is a moderate loss of dopamine in the nigrostriatal pathway, resulting in parkinsonian rigidity and bradykinesia. Co-existing Alzheimer's pathology (amyloid plaques) is present in >50% of cases and accelerates the decline.
Diagnostic Criteria & Guidelines
McKeith Criteria for Probable DLB: Dementia PLUS two or more core clinical features (fluctuating cognition, visual hallucinations, REM sleep behavior disorder, parkinsonism) OR one core feature PLUS one indicative biomarker (abnormal DAT scan).
For cognitive fluctuations and hallucinations: Cholinesterase inhibitors (Donepezil 5-10 mg daily or Rivastigmine patch 4.6-9.5 mg/24h). They are highly effective in DLB due to profound cholinergic deficits. For parkinsonism: Carbidopa/Levodopa (start low, e.g., 25/100 mg half-tablet BID) to improve mobility, but must be used cautiously as it can exacerbate hallucinations. For RBD: Melatonin 3-10 mg at bedtime.
Second-Line & Adjunctive Therapy
If hallucinations are severe, distressing, and unresponsive to donepezil: VERY cautious use of specific atypical antipsychotics with the lowest D2 affinity (Quetiapine 12.5-25 mg at bedtime or Pimavanserin 34 mg daily). For refractory RBD: Clonazepam 0.5 mg at bedtime (use with caution due to fall risk).
Surgical & Procedural Management
None. Deep Brain Stimulation (DBS) is contraindicated in DLB due to the high risk of worsening cognitive decline.
Patient Counseling & Advice
Thoroughly educate the family that the patient is NOT 'faking' their good or bad days—fluctuations are a core feature of the brain disease. Explicitly warn the family and document in the chart: THE PATIENT MUST NEVER BE GIVEN HALDOL OR TYPICAL ANTIPSYCHOTICS in an emergency room or hospital setting.
Follow-Up & Monitoring Schedule
Frequent outpatient monitoring to balance the "teeter-totter" of treatments (e.g., levodopa helps movement but worsens hallucinations; antipsychotics help hallucinations but destroy movement). Monitor orthostatic blood pressures.
Preventive Strategies
No known preventative measures exist.
Progressive and incurable. Progression is generally more rapid than Alzheimer's disease. Median survival is roughly 5 to 8 years from diagnosis, often terminating in complications like aspiration pneumonia or fatal falls.
