A focal, intracerebral infection that begins as a localized area of cerebritis and develops into a collection of pus surrounded by a well-vascularized capsule. It primarily affects the brain parenchyma and can occur in any lobe, most commonly the frontal and temporal lobes. It is a life-threatening emergency requiring prompt medical and often surgical intervention.

Detailed Overview

A brain abscess forms when pathogens (bacteria, fungi, or parasites) enter the brain tissue, typically through contiguous spread from adjacent structures (e.g., otitis media, sinusitis, dental infection), hematogenous spread from a distant focal infection (e.g., endocarditis), or direct trauma/neurosurgery. As the immune system walls off the infection, a capsule forms, leading to a space-occupying lesion that increases intracranial pressure (ICP) and causes focal neurologic deficits and seizures.

Epidemiology & Demographics

Incidence is approximately 0.3 to 1.3 cases per 100,000 person-years. The median age of presentation is 30-40 years. Males are affected more frequently than females (ratio of 2-3:1). More common in immunocompromised individuals and those in developing nations.

Etiological Mechanism

Most commonly polymicrobial. Frequent pathogens include Streptococcus species (especially S. anginosus group), Staphylococcus aureus (especially in trauma/surgery), Bacteroides species, and Enterobacteriaceae. Fungi (Aspergillus, Candida) and Toxoplasma gondii are common in immunocompromised hosts.

Primary Causes

Pathogenesis proceeds in four stages: 1) Early cerebritis (days 1-3) with local inflammatory infiltrate and edema; 2) Late cerebritis (days 4-9) with central necrosis and maximal edema; 3) Early capsule formation (days 10-13) characterized by neovascularization and a collagenous ring enclosing the necrotic center; 4) Late capsule formation (days 14 and beyond) with a well-developed, dense collagen capsule and surrounding gliosis. The expanding mass effect and vasogenic edema cause elevated ICP and tissue shift.

Diagnostic Criteria & Guidelines

Diagnosis is established by contrast-enhanced neuroimaging (MRI preferred) showing a ring-enhancing lesion with surrounding edema, combined with stereotactic aspiration or biopsy confirming pus and identifying the causative organism via Gram stain and culture. Lumbar puncture is absolutely contraindicated due to herniation risk.

First-Line Treatment:

Empiric intravenous antibiotics. For immunocompetent adults without neurosurgery: Ceftriaxone 2 g IV q12h (or Cefotaxime) PLUS Metronidazole 500 mg IV q8h PLUS Vancomycin 15-20 mg/kg IV q8-12h (trough target 15-20 mcg/mL). Total duration is 6-8 weeks based on imaging response. Dexamethasone 10 mg IV load then 4 mg IV q6h ONLY if significant mass effect or midline shift is present, tapered quickly.

Second-Line & Adjunctive Therapy

For post-neurosurgical/trauma: Vancomycin PLUS Cefepime 2 g IV q8h (or Meropenem 2 g IV q8h) to cover MRSA and Pseudomonas.

Surgical & Procedural Management

Stereotactic aspiration is the gold standard for diagnosis and treatment. Indicated for abscesses >2.5 cm, causing mass effect, or proximity to ventricles. Craniotomy and excision is indicated for multiloculated abscesses, post-traumatic abscesses containing foreign bodies, or failure of multiple aspirations.

Patient Counseling & Advice

Inform the patient that recovery may take months. Long-term antiepileptic drugs (e.g., Levetiracetam 500-1000 mg BID) may be required for at least 3-6 months. Discuss the risk of permanent neurologic deficits and the absolute necessity of completing the full 6-8 week course of IV antibiotics.

Follow-Up & Monitoring Schedule

Weekly clinical evaluation. MRI or CT Brain with contrast every 1-2 weeks during treatment, then monthly for 3-6 months after antibiotic completion to ensure complete resolution of enhancement and no recurrence.

Preventive Strategies

Prompt treatment of contiguous infections (otitis media, sinusitis, dental caries). Antibiotic prophylaxis prior to dental procedures in patients with cyanotic congenital heart disease.

With modern neuroimaging and surgical techniques, mortality has decreased to 5-15%. However, neurologic sequelae (hemiparesis, seizures, cognitive deficits) occur in 20-50% of survivors.
