Bernard-Soulier syndrome (BSS) is a rare, inherited bleeding disorder characterized by thrombocytopenia, giant platelets, and severe platelet dysfunction due to a defect in the glycoprotein (GP) Ib/IX/V complex on the platelet surface.
Detailed Overview
The GPIb/IX/V complex is the primary receptor for von Willebrand factor (vWF). Without it, platelets cannot adhere to the subendothelium at sites of vascular injury. Patients present with severe mucocutaneous bleeding that is out of proportion to their moderately low platelet count. It is an autosomal recessive disorder.
Epidemiology & Demographics
Extremely rare, with an estimated prevalence of less than 1 in 1,000,000. More common in populations with high rates of consanguinity.
Etiological Mechanism
Mutations in the GP1BA, GP1BB, or GP9 genes, leading to absent or dysfunctional GPIb/IX/V complexes.
Primary Causes
Autosomal recessive inheritance of mutated genes encoding the platelet GPIb/IX/V receptor.
Normal primary hemostasis requires platelets to adhere to exposed subendothelial von Willebrand factor (vWF) via the GPIb/IX/V receptor. In BSS, this receptor is missing or defective. Consequently, platelets fail to tether to the injured vessel wall under high shear stress conditions. Additionally, megakaryocytes release abnormally large (giant) platelets, and overall platelet survival is slightly decreased, causing mild to moderate thrombocytopenia.
Diagnostic Criteria & Guidelines
Diagnosis requires the demonstration of giant platelets on peripheral smear, failed platelet aggregation with Ristocetin (which is NOT corrected by adding normal plasma, differentiating it from vWD), and normal aggregation with ADP, epinephrine, and collagen. Confirmed by flow cytometry showing absent/decreased CD42a/b (GPIb/IX).
Local hemostatic measures (e.g., manual pressure, Gelfoam). Antifibrinolytics for mucosal bleeding: Tranexamic acid 1300 mg PO TID or Aminocaproic acid. Recombinant factor VIIa (rFVIIa) 90 mcg/kg IV for severe bleeding episodes.
Second-Line & Adjunctive Therapy
Platelet transfusions (HLA-matched, leukocyte-depleted if possible) reserved strictly for severe, life-threatening bleeding or major surgery due to the high risk of alloimmunization to the GPIb/IX complex.
Surgical & Procedural Management
Hematology consultation required prior to any surgery. Pre-operative preparation with rFVIIa or platelet transfusion is usually necessary.
Patient Counseling & Advice
Educate on the critical nature of informing all healthcare providers (including dentists) about their diagnosis before any procedures. Recommend wearing a medical alert bracelet.
Follow-Up & Monitoring Schedule
Routine hematology follow-up every 6-12 months. Monitor CBC for iron deficiency anemia.
Preventive Strategies
Genetic counseling for family planning. Prevent bleeding via lifestyle modifications.
Generally good with appropriate management, though severe, fatal hemorrhagic episodes can occur. Quality of life can be impacted by recurrent bleeding.
