Core Definition:

An aspergilloma is a conglomeration of intertwined Aspergillus fungal hyphae, fibrin, mucus, and cellular debris that forms a spherical mass (fungus ball) within a pre-existing cavitary lesion in the lung parenchyma. It is a form of chronic pulmonary aspergillosis that occurs in immunocompetent hosts with structurally damaged lungs.

Detailed Overview

Unlike invasive pulmonary aspergillosis which invades tissue in immunocompromised patients, an aspergilloma acts as a saprophyte. Aspergillus spores are inhaled and colonize an already dead-space cavity—most commonly left behind by prior pulmonary tuberculosis. The fungus grows freely inside this cavity but does not invade the surrounding lung wall. However, the mass can roll around with patient movement, causing chronic friction and inflammation on the cavity wall. This inflammation recruits fragile neovascularity from the systemic bronchial circulation, which is prone to rupture, leading to life-threatening hemoptysis.

Epidemiology & Demographics

Prevalence aligns with regions of high tuberculosis endemicity. Often found in adults with a history of treated TB or severe cavitary lung diseases like sarcoidosis or bullous emphysema.

Etiological Mechanism

Colonization by Aspergillus species, predominantly Aspergillus fumigatus (over 80% of cases), inside a pre-existing structural lung defect.

Primary Causes

The primary requirement is an anatomical dead space in the lung with communication to the tracheobronchial tree, allowing inhaled spores to enter.

Inhaled Aspergillus conidia (spores) settle into a non-ventilated, poorly perfused cavity. Evading the immune system due to poor local blood supply and dead space, the conidia germinate into hyphae, creating a dense mycelial mass mixed with host tissue debris. The fungal ball is typically unattached and mobile. Aspergillus secretes proteolytic enzymes and antigenic toxins that cause chronic localized inflammation of the cavity wall. This triggers hypertrophy of the nearby bronchial arteries to supply the inflamed tissue. These collateral vessels are highly tortuous, thin-walled, and under systemic arterial pressure, making them highly susceptible to rupture and massive hemoptysis.

Diagnostic Criteria & Guidelines

Diagnosis is based on 1) Radiographic evidence of a round mass within a lung cavity; 2) Microbiological or serological evidence of Aspergillus (positive Aspergillus IgG precipitins).

First-Line Treatment:

For asymptomatic simple aspergillomas, observation and routine follow-up are sufficient. Systemic antifungals have poor penetration into cavities. For patients with recurrent or significant hemoptysis, Bronchial Artery Embolization (BAE) is the first-line emergent intervention to stop bleeding by injecting thrombogenic material into the hypertrophied vessels. BAE serves as a bridge to surgery or a definitive treatment for poor surgical candidates.

Second-Line & Adjunctive Therapy

For simple aspergilloma causing severe hemoptysis in a patient with adequate pulmonary reserve, surgical resection (lobectomy or segmentectomy) is the definitive cure. If surgery is high-risk, long-term oral triazoles (Itraconazole 200 mg BID or Voriconazole 200 mg BID for 4-6 months) may be attempted to prevent progression to complex CPA, though they rarely eradicate the fungus ball.

Surgical & Procedural Management

Thoracoscopic or open Lobectomy. Surgery carries a high risk of complications (bronchopleural fistula, empyema) due to dense pleural adhesions from prior TB, requiring a highly skilled thoracic surgeon.

Patient Counseling & Advice

Educate the patient that the 'ball' itself isn't cancer, but rather a mold sitting in an old scar. Emphasize that coughing up a teaspoon of blood is a warning sign that requires an immediate ER visit, as it can escalate to life-threatening bleeding suddenly.

Follow-Up & Monitoring Schedule

Asymptomatic patients require annual chest CT and clinical evaluation to ensure stability of the lesion and absence of hemoptysis.

Preventive Strategies

Prompt and complete treatment of primary cavitary diseases (like Tuberculosis) to minimize residual dead space in the lungs.

Variable. Asymptomatic simple aspergillomas may spontaneously regress in ~10% of cases or remain stable for life. Symptomatic lesions causing massive hemoptysis carry high mortality if not intervened upon promptly.
