Core Definition:

Anti-glomerular basement membrane (anti-GBM) disease is a rare, life-threatening autoimmune vasculitis characterized by circulating autoantibodies directed against the non-collagenous domain of the alpha-3 chain of type IV collagen. It causes rapidly progressive glomerulonephritis (RPGN) and alveolar hemorrhage (pneumonitis). When both lung and kidney are involved, it is traditionally called Goodpasture Syndrome.

Detailed Overview

Anti-GBM disease is a classic Type II hypersensitivity reaction. The autoantibodies specifically attack type IV collagen, which is heavily concentrated in the basement membranes of the renal glomeruli and pulmonary alveoli. This leads to profound complement activation and tissue destruction, causing acute kidney failure and severe bleeding into the lungs. Prompt diagnosis via serology and biopsy, followed by aggressive treatment with plasmapheresis and immunosuppression, is critical to prevent permanent organ failure or death.

Epidemiology & Demographics

Extremely rare, incidence of 1-2 cases per million per year. Bimodal age distribution: young men (20-30s) often present with both lung and kidney involvement, while older adults (60-70s) often present with isolated kidney involvement. Highly associated with HLA-DR15.

Etiological Mechanism

Autoimmune. An environmental trigger (e.g., viral infection, smoking, hydrocarbon inhalation) in a genetically susceptible host (HLA-DR15) exposes the sequestered alveolar/glomerular antigens, leading to IgG autoantibody production.

Primary Causes

IgG autoantibodies bind to the alpha-3 chain of type IV collagen in the glomerular basement membrane (GBM) and alveolar basement membrane. This binding activates the classical complement pathway and recruits neutrophils and macrophages. In the kidneys, this causes crescentic glomerulonephritis with rapid destruction of nephrons. In the lungs, it damages the alveolar-capillary barrier, leading to intra-alveolar hemorrhage and severe impairment of gas exchange.

Diagnostic Criteria & Guidelines

Diagnosis is confirmed by the presence of circulating Anti-GBM antibodies AND compatible biopsy findings (usually renal biopsy) demonstrating linear IgG deposition along the GBM on immunofluorescence.

First-Line Treatment:

A life-saving, three-pronged immediate approach: 1) Plasmapheresis (therapeutic plasma exchange) daily for 14 days to rapidly remove circulating antibodies. 2) High-dose Corticosteroids (e.g., IV Methylprednisolone 1g daily for 3 days, then oral Prednisone 1 mg/kg/day) to halt inflammation. 3) Cyclophosphamide (e.g., 2 mg/kg/day PO or IV pulses) to stop new antibody production.

Second-Line & Adjunctive Therapy

Rituximab (anti-CD20 monoclonal antibody, e.g., 375 mg/m2 weekly x 4) is increasingly used alongside or instead of cyclophosphamide to deplete B-cells. If patient progresses to ESRD despite therapy, hemodialysis is required.

Surgical & Procedural Management

Kidney transplantation can be performed for patients with resulting ESRD, but strictly ONLY after anti-GBM antibodies have been completely undetectable in the serum for at least 6 months, to prevent the disease from destroying the new kidney.

Patient Counseling & Advice

Counsel on the high risk of permanent kidney failure even with optimal treatment. Discuss the severe immunosuppressive risks of cyclophosphamide (infection, infertility, hemorrhagic cystitis).

Follow-Up & Monitoring Schedule

Weekly monitoring of anti-GBM titers, serum creatinine, and CBC until remission. Then monthly for several months. Relapses are rare (<5%) compared to ANCA vasculitis.

Preventive Strategies

No primary prevention exists, but smoking cessation is crucial to prevent pulmonary involvement in susceptible individuals.

With aggressive early treatment, survival is >90%. However, if serum creatinine is >5.7 mg/dL or >50% crescents are seen on biopsy at presentation, the chance of recovering independent kidney function is extremely low (<10%). Lung recovery is usually complete.
