Actinomycosis is a rare, subacute to chronic bacterial infection characterized by contiguous spread, suppurative and granulomatous inflammation, and formation of multiple abscesses and sinus tracts that may discharge sulfur granules. It is primarily caused by Actinomyces species, which are normal flora of the oral cavity, gastrointestinal tract, and urogenital tract. The infection most commonly affects the cervicofacial region.

Actinomycosis is an endogenous infection that typically occurs following mucosal disruption, such as from dental extraction, trauma, or surgery. The bacteria, predominantly Actinomyces israelii, are anaerobic or microaerophilic Gram-positive bacilli that do not respect tissue planes during infection. They form rock-hard, indurated masses with woody fibrosis, leading to tissue destruction and fistula formation.
Epidemiology & Demographics
Actinomycosis is rare, with an estimated incidence of 1 in 300,000 per year. It is more common in males (3:1 ratio) and typically affects individuals aged 20 to 60 years. Cervicofacial disease accounts for 50-60% of cases, abdominopelvic 20%, thoracic 15%.
Etiological Mechanism
Caused primarily by Actinomyces israelii, with other species like A. naeslundii, A. viscosus frequently involved. Infections are usually polymicrobial, involving companion bacteria such as Aggregatibacter actinomycetemcomitans.
Primary Causes
Mucosal barrier disruption allowing normal flora to invade deep tissues, commonly resulting from poor dental hygiene, dental procedures, abdominal surgery, or prolonged intrauterine device (IUD) use.
Following mucosal injury, Actinomyces species invade deep tissues in a low oxygen environment. They form complex polymicrobial clusters that resist phagocytosis. The infection spreads slowly, ignoring fascial planes, triggering a chronic granulomatous and suppurative inflammatory response. This leads to intense local fibrosis ('woody induration') and central necrosis, eventually breaking through the skin or mucosa to form sinus tracts that extrude yellowish clumps of bacteria and calcium phosphate known as 'sulfur granules.'
Diagnostic Criteria & Guidelines
Definitive diagnosis requires isolation of Actinomyces species from a sterile site or typical histopathological findings of 'sulfur granules' (basophilic masses with eosinophilic clubs on H&E stain) in the setting of compatible clinical signs.
Penicillin G 18-24 million units IV daily for 2-6 weeks, followed by Amoxicillin 500 mg PO TID for 6-12 months.
Second-Line & Adjunctive Therapy
Doxycycline 100 mg PO BID or Ceftriaxone 2 g IV daily, followed by Clindamycin 300 mg PO QID for 6-12 months.
Surgical & Procedural Management
Surgical resection and drainage if abscesses are large or for relief of obstruction.
Patient Counseling & Advice
Inform the patient that treatment is very long (up to a year) to ensure total eradication and prevent relapse.
Follow-Up & Monitoring Schedule
Monthly clinical review initially, followed by regular imaging to ensure the mass is resolving.
Preventive Strategies
Optimal oral hygiene, timely treatment of dental caries, limit IUD placement duration to under 5 years.
Cure rates are over 90% with prolonged antibiotic therapy, although residual anatomical deformity can remain.
